Description:
This workshop is designed to define what a change of condition is in the residents that we care for and the steps to appropriately handle a change of condition. A resource manual is provided that has assessment tools that can be used to assist in this process.

Objectives:

• Understand and comprehend the definition of change in condition.

• Understand the process involved with each step of effective change of condition management that includes:

a. Assessments
b. Reporting
c. Interventions
d. Documentation
e. Follow-up
f. Monitoring

• Understanding the IDT members role in Change of Condition Management
• Understand the tools that are available for Change of Condition Management

Prerequisites:

The program is designed specifically for Licensed Nurses. However, other members of the Interdisciplinary team are welcome.

Program Format:
The format of this course is interactive. Your participation is expected throughout the lectures and demonstration.
